Specialty Referral Project

The goal of this project is to foster effective and efficient referrals processes in the local medical community. The project provides specialty-specific tools for you to use.

This project started as a collaboration between the Northcoast Association of Advanced Practice Clinicians and the Humboldt Quality Collaborative of the Humboldt-Del Norte Foundation for Medical Care/Independent Practice Association.
